Glenns Ferry Health Center, Inc.  is currently hiring for the following positions:

SUBSTANCE USE DISORDER CLINICIAN

    This individual is principally responsible for intervening on behalf of children, adolescents, and adults who have suffered neglect, physical, sexual, and/or emotional abuse, or suffer from emotional disturbances or mental illness. At least 50% of the case load will involve clients with substance use disorders. The LMSW/LCSW/LCPC/LPC is responsible for assessing, and if appropriate, treating and connecting these individuals in need of services to appropriate departments, programs, and/or to other agencies and sources around the community which could meet the individual needs of the patient. 

The LMSW/LCSW/LCPC/LPC for this position will have experience working with substance use disorders; Two or more years of previous direct client treatment; and previous experience with health care preferred. If you meet these qualifications

FULL TIME  Care Coordinator 

Assure the delivery of comprehensive care coordination services to Glenns Ferry Health Center (GFHC) patients and their families.  Be responsible for accurate and complete documentation of service to patients and families.  Develop in- depth knowledge of and positive relations with existing community resources.  Works as part of the clinical team and is responsible to make contact with patients by letter, phone, and/or portal to address follow-up care.  Collaborates with other health team members assisting in the promotion of the health and well-being of patients.

Great organizational skills, and team building is a must.  Bilingual (English/Spanish) preferred but not required and some travel required. Preferred experience working with: substance abuse

disorders, direct client treatment in a healthcare setting, individual assessments, interventions,

family and group therapy. 

 

Baccalaureate/Associates degree in Public Health or in appropriate health field or a combination of education

and experience preferred.

Experience with outreach activities preferred.

Knowledge and experience with case management approach.

Experience with interagency coordination.

Bicultural/bilingual or experience working with Hispanic population.

Experience working in agencies involved in research and demonstration projects funded by grants.
